Since 1972, we have assisted older and disabled adults to continue to live with dignity and independence in the residential settings of their choice. They receive:
- Professional in-home assessment of needs
- Consultation with all involved parties
- Personalized plan of care
- Arrangement and coordination of needed services
- Advocacy to obtain medical care and benefits
- Ongoing monitoring of services and participant needs
- Assistance in planning alternative living arrangements
- Arrangement for other in-home services such as financial management, home care, housekeeping, personal care, transportation and yard work.
We also provide: service coordination and linkage to benefits to the residents of the City of Tucson’s subsidized housing complexes, and grocery shopping services for people who have difficulty shopping for themselves.
Private Care Options
Sooner or later, we all need a little help to live with dignity and independence in our own homes. Most of us do not qualify for publicly funded programs, and navigating the maze of care options can be intimidating. Our Family offers affordable assistance to keep older and disabled adults safe and well, while easing the burdens on those who make decisions about their care. Services & Fees
Brief Assessment — $150 (up to 2 hours ; $75 each additional hour)
One-time in-home review of client’s current problems and options. Oral recommendation and list of options for care will be provided, but no formal, written care plan.
Comprehensive Assessment & Care Planning — $250 (up to 3 hours)
Full assessment of living environment, physical and psychological barriers to independence, lifestyle and financial resources. After identifying current and potential problems, options are presented and formal care plan is developed, including advice in finding appropriate housing placement.
Ongoing Case Management — $75 per hour (minimum 2 hours monthly)
Regular home visits monitor the client’s well-being and ensure services are being delivered. We can arrange for and monitor in-home services provided at extra cost by private agencies (such as house cleaning, laundry, help with bathing, meal preparation, errands and transportation) and can arrange for and accompany clients to doctors’ appointments.
Grocery Shopping — $30 per hour
Our staff picks up groceries and prescriptions from Fry’s and delivers to your home.
Counseling — $75 per hour
Individual, couple and family sessions in the home or our offices may focus on grief, adjustment to disability, relocation, trauma and family issues.
